  • Are there out-of-network options?
    Yes. Opticare offers many plan design options that have an out-of-network allowance that directly reimburses employees.

  • Will my members receive an insurance card?
    Yes. Each member will receive (2) cards outlining your plan ID, Member ID, Group ID and Effective Date. They will also receive a Summary Plan Description and a Certificate of Coverage.
