Is Opticare an Insurance Company?
Yes, Opticare of Utah is an LHMO (Limited Health Plan), and operates under the auspices of the Utah Department of Insurance.
Can Opticare cover non-Utah groups?
Yes. Opticare of Utah can cover employees and members in any state in the US.
Are there out-of-network options?
Yes. Opticare offers many plan design options that have an out-of-network allowance that directly reimburses employees.
How do I get a quote from Opticare for my groups?
Call 1-800-EYECARE and ask for the sales department.

A service representative will return your quote in no less than 24 hours.
Will my members receive an insurance card?
Yes. Each member will receive (2) cards outlining your plan ID, Member ID, Group ID and Effective Date. They will also receive a Summary Plan Description and a Certificate of Coverage.
