Sure you want a vacation, but you’ve been working too much, right? Well, now your hard work will force you to take it easy. Presenting Opticare of Utah’s Excellence Riviera Cancun incentive getaway. Qualifying agents and agencies will enjoy 5 nights at an all inclusive resort in beautiful Mexico. The trip departs from Salt Lake on February 2, 2012 and returns February 7, 2012. Goals, Guidelines, Rules

Renewal enrollments will be given a 20% credit, new group enrollments count 100%.
Effective Date: January 1, 2011 - December 21, 2011.

750 Lives equals a Trip for (2)

Qualifications

Net new lives include new group enrollments as well as net new additions to existing groups.
New enrollments on existing accounts and re-enrollments are calculated at net, and will be credited at 20%.
Net new lives will be adversely affected by terminated or lost groups.
New Net Lives means contracts, not including dependents.
