| Enrolling
with Opticare is as easy as selecting the plan that your employer provides,
or if you are an individual, select the plan you wish to participate on.
Both group and individual enrollment forms must be printed, filled out
completely and returned via mail or fax to Opticare of Utah. Once your
enrollment is received, your account will be activated and you will
receive an enrollment card in the mail in just a few days. |
