As an existing member of Opticare of Utah, this page allows you to not only view the plan you are enrolled in, but also view the other options that your employer may offer. By choosing “Select a Plan” and choosing your employer, you can view these options, and also print PDF copies of your plan design. If you cannot find you employer on this page, please call you HR or payroll department or Opticare of Utah Customer Service at 1-800-EYE-CARE.
Remember, for most group plans, current members can only make changes, additions or deletion during a qualifying event – marriage, birth, death, divorce, etc – or during your company's open enrollment period. Direct these request to your employer.
