| As a new
member of Opticare of Utah, you are now authorized to view the plan options
your employer provides. In many instances, you can print an enrollment
form and return to you payroll or human resources department immediately.
If you are an individual wishing to sign up for a plan, you can do so on
this page as well. For comments regarding which plan(s) to select, you
can call Customer Service at 800-EYE-CARE. |

For group or voluntary plans, please review the plan options and make your selection.
Choose “Enrolment Forms” and print the appropriate form for the plan you wish
to select. Fill out this form completely and return to Opticare of Utah via
fax, 801-954-0054 or mail. When your application is accepted – usually just
a few days – you will receive your enrollment card in the mail. Once your card
is received, you may obtain benefits immediately.
