Opticare 15-100C Individual Plan
This plan is designed for the self-employed or those who are not offered an ideal vision plan through their employer. For ease of understanding, 15-100C is a plan with a $15 exam co-pay and $100 frame and contact lens allowance. This plan not only uses the Standard Optical Select Network but also gives you access to over 40 independent Optometrist who are credentialed on our Broad Network and out-of-network options, giving you the ability to go anywhere you want while still getting great benefits.
