| Opticare of Utah offers plan designs to satisfy all your vision plan needs. Evaluating network options is a great way to keep the value premium rates for your groups while having a rich benefit option. For example, plans Basic and Premier utilize the Standard Optical Provider Network only. Opticare has negotiated preferential reimbursements which make these plans our most affordable options. On the other hand, for many groups, network accessibility and selection are a determining factor in which vision plan to choose. 10-70 and 10-120 are the plans for you if broader network selection is a priority for your group. Opticare Basic
This plan is our most affordable option - $10 exam co-pay, $70 frame allowance and no materials co-pay, therefore basic plastic lenses are covered 100%. Also, up to $50 are covered for contact lenses. This plan is offered with the Standard Optical network only and represents one of our most popular options for premium affordability.
Opticare Premier
This plan utilizes the Standard Optical preferred network and offers a richer
option than the BASIC Plan. This plan differentiates itself on the frame allowance
and contact lens allowance benefits. Frame and contact lens allowance of $100
retail. This is a great plan when affordability is a concern, but a richer framer
allowance is desired.
Opticare 10-70C
This plan is designed for broader network accessibility and affordability. For
ease of understanding, 10-70C is a plan with a $10 exam co-pay and $70 frame
and contact lens allowance. This plan uses not only the Standard Optical Network
but also over 40 independent Optometrist who are credentialed on our Expanded
panel.
Opticare 10-120C
This is our most popular plan option with $10 exam co-pay and $120 frame and
contact lens allowances. This is the ideal plan for comprehensive benefit options,
broad network accessibility and affordability. More employers select plan 10-120
than any other plan offering.

Opticare - State of Utah Employees
This plan is offered to PEHP State of Utah Employees only. Check out the link
below to learn more about 10-70C. Plan 10-70C has a different provider panel
than the other Opticare of Utah Options. If you or your groups are interested
in a plan such as 10-70C, look at Opticare Basic.
Value Vision
This is Opticare of Utah's preferred provider discount program. This program
is a reduced fee for service, discount program affording the member 35-50% off
retail pricing. Standard Optical providers offer this discount to contracted
providers such as Altius Health Plans, PEHP Plus, Regence Blue Cross
Blue Shield of Utah, Intermountain Health Care Employees. Check out the plan

Limitations and Exclusions
Safety Eyewear
Opticare of Utah offers Safety Eyewear programs to over 50,000 Utah employees.
Safety eyewear discount plans are available at no additional charge to Opticare
of Utah employer clients. Safety eyewear programs can save employers and employees
thousands of dollars each year. Clients include: Utah Kennecott Copper, EG&G
Defense Materials, Envirocare of Utah, Lifetime Products, Granite Construction

LASIK and Cataracts
More than 7000 patients have had LASIK Vision Correction with the professionals
at Standard Optical. Opticare offers our clients a discount of $100 OFF per
eye for all procedures and prescription ranges. This benefit is our most popular
value-added benefit and is available in the Standard Optical Preferred Provider
Network.
